Qualified DOT SAP Evaluation Services

All appointments conducted via telehealth. ODMHSAS and DOT approve certified/qualified providers to deliver these services via real-time telehealth video meetings.

Be sure to expand appointment options in each category, as you might find a preferred time slot within our “same day” or “prepayment” availability, including weekend appointments.

To receive your ADSAC Completion Certificate with the Red Stamp, submit your original recommendation requirement certificates via drop-off in the designated locked mailbox outside of our office, mail, or fax to (405) 259-3043. We’ll process and upload the certificate, along with scanned copies, to your client portal for easy download or printing.

Substance Abuse Evaluations for those employed in Department of Transportation safety-sensitive positions that fall under Federal Motor Carrier Safety Administration (FMCSA), Federal Aviation Administration (FAA), United States Coast Guard (USCG), Federal Railroad Administration (FRA), Pipeline and Hazardous Material Safety Administration (PHMSA) and Federal Transit Administration (FTA).

The total fee must be paid at the time of the initial evaluation and covers the cost of the initial and follow-up evaluation. This may be paid by the employee or the employer. There may be additional fees incurred for missed appointments which are the responsibility of the employee.

Substance Abuse Evaluations for individuals employed in public safety-sensitive roles, excluding those within agencies regulated by the Department of Transportation (DOT). These roles encompass a broad spectrum, including health professionals (such as doctors, nurses, pharmacists, therapists), construction workers, teachers, and first responders (EMT, firefighters, police officers, etc.). Substance Abuse Evaluations for individuals employed in public safety-sensitive roles, excluding those within agencies regulated by the Department of Transportation (DOT). The complete fee for the evaluation must be settled during the initial assessment and encompasses both the initial and follow-up evaluations. Payment can be made either by the employee or the employer. Any additional fees resulting from missed appointments will be the responsibility of the employee.

Comprehensive mental health evaluation designed to aid in the assessment, diagnosis and provision of treatment options/referrals. It is conducted by qualified professionals using empirically guided clinical instruments. It is often utilized for legal purposes, including court, attorney requests, probation, and DHS requirements.

The initial appointment is up to 90 minutes, and a follow-up appointment may be required. This will be determined and scheduled during the initial session. The service cost covers the initial evaluation, follow-ups, and the provider's time for the assessment report.

The full fee, including all appointments and the report, is due at the start of the initial assessment.

It's crucial to note that this assessment does not encompass an ADSAC evaluation, necessary for driver's license reinstatement. If you also require an ADSAC Assessment for license reinstatement, please schedule it as a separate service.

Please note that this appointment displayed on our booking site for the Completion Certificate and Red Stamp is for informational purposes only. If you require your Completion Certificate and Red Stamp, please follow the aforementioned instructions and also refrain from booking a new appointment for assessment. Booking a new assessment appointment will result in a charge to you for the full fee for a new assessment as it prevents others who genuinely need a new assessment from accessing the 1.5 hour available time slot.

If your assessment was not conducted through ADSAC OKC, please reach out directly to the assessor that conducted your assessment for information regarding your assessment. ADSAC OKC can only issue Completion Certificates and the Red Stamp for assessments conducted by our assessors.
